Safety, Housing Support, Financial Aid,

One resident supported through the partnership has made significant progress over the past few months after facing a period of instability and vulnerability. Through consistent outreach, welfare checks, and patient engagement from a range of local services, the individual was able to build trust with support staff and begin addressing some of the challenges that were affecting their wellbeing and day-to-day circumstances. There were concerns around personal safety, the people they were associating with, and the impact these issues were having on their mental health. By maintaining regular contact and offering a supportive, non-judgemental approach, staff were able to help the individual stay engaged and connected to support during a particularly difficult period.

A key focus of the work was improving safety and helping the individual move towards a more stable living situation. Services worked together to identify suitable accommodation, reduce immediate risks, and provide practical support throughout a period of transition. Alongside this, staff helped the individual access essentials such as food, clothing and household items through emergency assistance and external funding opportunities. Support was also provided to connect them with specialist advice services, including assistance to explore benefit entitlements and improve financial stability.

Since these interventions, the individual has reported feeling more settled, secure and optimistic about the future. Regular welfare visits have continued to reinforce the positive progress made, while access to practical, financial and emotional support has helped them regain stability and confidence. The individual is now engaging well with support services, has successfully accessed additional income and resources, and is in a much more positive position than when support first began. This case highlights the difference that partnership working, persistence and person-centred support can make in helping someone overcome challenges and build a more stable future.


Employability, Volunteering and social inclusion.

One resident was supported following a move into the area and was finding it difficult to settle in due to being unfamiliar with local services and opportunities. Working alongside partner agencies, support was provided to help them become more familiar with their new surroundings and identify opportunities that matched their interests, strengths and future goals.

As part of this work, the resident was introduced to a range of community activities and services, including volunteering opportunities, employability support and wellbeing groups. Time was taken to visit local venues, explain the support available and build confidence in accessing these independently. Practical issues identified during visits were also addressed to help the resident settle comfortably into their new home.

The resident was particularly interested in employability and volunteering opportunities and was pleased to learn that dedicated support was available locally. They spoke positively about having access to guidance and encouragement to help them explore their options and work towards their goals. Through engagement with local services and community groups, the resident identified a number of opportunities they would like to pursue, helping to build confidence, develop social connections and support their longer-term independence and involvement in the community.


Health and Wellbeing, Social Care

One resident supported through the partnership has seen significant improvements in their health and wellbeing through sustained engagement with local support services. Initial welfare visits identified a range of complex health issues that were affecting their daily life, independence and overall quality of life. Building a positive relationship with the resident allowed staff to better understand their circumstances and support them to reconnect with services that they had previously struggled to access.

A key priority was improving access to healthcare. Through ongoing practical support and advocacy, the resident was assisted to attend multiple GP appointments and engage with health professionals to address a number of long-term medical conditions. This led to a comprehensive review of their health, including further investigations and hospital appointments where required. Staff worked closely with the resident to reduce barriers to attending appointments, ensuring they received the medical attention and follow-up support they needed.

Alongside health interventions, work also focused on improving the resident's safety, wellbeing and ability to live independently. Support was provided to reconnect them with social care services, explore options for additional help at home, and put measures in place to protect them from financial exploitation. Practical assistance, regular wellbeing visits and improved communication methods helped the resident stay connected to services and better manage their day-to-day needs.

As a result of this coordinated support, the resident is now much better connected to healthcare and wider support networks than before. They have undergone important health assessments, are receiving ongoing input from relevant professionals, and have additional safeguards in place to support their independence. This case demonstrates the positive impact that persistent, person-centred support can have in helping someone access the right care, improve their wellbeing and feel more secure in their everyday life.
