Consent and Confidentiality Leaflets and Form

Please find below the Information Sharing Leaflet for parents, z-card for young people, consent form and practitioner's guidance.

There are also copies available within the County Buildings for Education, Whitletts and Girvan Social Work Offices and North Ayr and Girvan Health Centres.

The Z-Card should be given to all young people involved in an Integrated Assessment as soon as it is commissioned. The Parents Leaflet should be given to all those involved in integrated assessment before consent is requested for sharing information. It is good practice not to request consent if the case is Child Protection and alerting the parent/carer may endanger the child further.
